Discuss the clinical need.

Discuss occupational, physical, speech and massage therapy, counseling, nursing, attendant care or specialized evaluations. Share the goals of the referral so the requested services can be reviewed together.

Prepare for the referral conversation.

• Patient location and the disciplines being requested.
• The clinical question, functional concerns and current care plan.
• Relevant orders and documentation, as requested by the office.
• The people involved in coordinating the patient’s care.
