Clinical needs belong in the care plan.

Nursing needs may involve assessment, monitoring, education, medication-related support, and other clinically appropriate services. The requested service should reflect the treating plan, current orders, and the patient’s needs.

Explain what has changed, what care is being requested, and which clinical contacts are involved. The office can identify the information needed to review a referral; it cannot establish coverage or accept a case through this page alone.

Prepare useful referral information.

• Patient location and discharge or transition plans.
• Current clinical concerns and requested nursing services.
• Available orders, care instructions, and relevant records.
• Treating-provider contacts and other services involved.
• Questions about scheduling, caregiver education, and payment arrangements.

Coordinate nursing and rehabilitation.

A person may have nursing needs alongside occupational therapy, physical therapy, speech therapy, or attendant care. Each service has its own role. Clear communication helps the treating team understand current needs and avoid conflicting instructions.

Ask which provider should receive questions about symptoms, medications, or a change in function. Nursing service arrangements do not replace emergency care.
