A home modification evaluation considers how injury affects function within the physical environment. The evaluation connects the person’s needs with the barriers encountered in the home.

- Access to and movement through the home.
- Daily activities and use of key living spaces.
- Safety barriers related to functional limitations.
- Equipment and environmental recommendations.
- Clinical documentation of identified needs.
What to expect.
The evaluator considers the person, injury, home environment and everyday activities. Recommendations are individualized. An evaluation does not guarantee funding, construction approval or insurance reimbursement.
The care plan and availability are assessed for each referral. Ask the office about required documentation and payment arrangements.
